Dr. Gazaleh Says UnumProvident Not Paying On Disability

Dr. Shawn Gazaleh has filed suit against UnumProvident, claiming the firm has not paid off on his disability claim.

Dr. Gazaleh, who at one time headed the emergency room at Erlanger Medical Center, said he has been paying $6,000 per month on three disability policies.

The lawsuit in Chancery Court says UnumProvident has not paid claims to him in many years.

The suit says his condition "is worsening day by day and each flareup is worse than the prior one." The suit does not state the nature of the ailment.

It says Dr. Gazaleh has not been able to get the therapy and medication he needs.

Chancellor Howell Peoples set a hearing in the case for Oct. 14 at 9 a.m.

Attorney Ronnie Berke filed the suit.
